Leading Cardiac Electrophysiologist Dr. T. S. Kler
First Time in India, Pacemaker via His Bundle Pacing: Leading Cardiac Electrophysiologist Dr. T. S. Kler Conducts Breakthrough Procedure
The procedure, that imitates the physiological pacing of the heart, is an improvement over traditional pacemaker implantation

The procedure marks a breakthrough not just in India, but also the SAARC region

New Delhi, Nov 27, 2014: In a major breakthrough in the field of Cardiac Electrophysiology in India, leading cardiologists at Fortis Escorts Heart Institute & Research Centre have performed the country’s first pacemaker implant via His Bundle pacing, a technique that saves more lives in the longer run as against the traditional implant procedure.

A team led by Dr T S Kler, HOD Cardiology and Director, Electrophysiology at Fortis Escorts Institute & Research Centre, performed the new procedure on two patients ,a 75-year-old man and another 63 years man ,who needed a pacemaker implant.

It is the first time that cardiologists in India have performed this procedure. In fact, this is the first time this technique has been executed anywhere in the SAARC region.

A human heart needs a pacemaker when heart rate drops to abnormally low lavels either because of inability of natural pulse generater center ( sinus node ) or when there is electrical block of impulse when it travels from right upper chamber to right lower chamber usually at the AV node or below that. An artificial pacemaker replaces the heart’s original electrical impulse generating system with an alternate power generator.

In traditional pacemaker implant, the lead of the pacemaker (or its wires) is attached to the right ventricle apex, a point located at the bottom of the lower right chamber of the heart. The electrical input then travels all the way from this lowest point in the heart upwards towards the base. Normal impulse travels from base of heart to its apex. So in apical pacing electrical impulse travels just the opposite to natural way. A section of people (About 10 to 15 % ) who undergo pacemaker implant through this traditional mode suffer from heart failure due to decrease in pumping capacity of heart caused by right ventricle apical pacing.

The new technology involving pacing through His Bundle pacing cuts this risk of future heart failure to almost zero % . In this breakthrough technique, which is performed by very few doctors across the world, the pacemaker lead is put on directly on the His bundle, which is the junction between the upper and lower chambers of the heart. This replicates the physiological functioning of the heart as the heart’s natural electrical impulses travel in normal human heart.

“In normal heart functioning, the electricity generated in the upper right chamber of the heart travels to the A-V node (atrioventricular node) & His bundle ,which is the bridge between the upper and lower chambers of the heart. This impulse then divides and travels to the left ventricle and the right ventricle. So, naturally speaking, AV Node & His bundle is the point that acts as a natural conductor of electricity from upper chambers to lower chambers of heart. When the AV node is blocked due to a malfunction, the electricity supply interrupts and this necessitates an artificial pacemaker,” says Dr Kler.

“In traditional pacemaker pacing, which is the norm across the world, there is a long-term risk of patients suffering from heart failure. In fact, 10 to 15 % people in traditional pacing do tend to suffer from heart failures in the longer run. The alternate His Bundle pacing is the physiological way of pacing and therefore cuts this risk,” adds Dr Kler.

After conducting the first such two procedures, the doctors are now preparing for a third His Bundle pacing procedure.

Right ventricular apical pacing – as the traditional method is called – is not an ideal procedure for permanent pacing because of a risk for ventricular function deterioration, mitral valve dysfunction, or proarrhythmic effects. His Bundle pacing provides a safer and better technique of providing electricity to lower chambers of heart. But only problem is it is technically more demanding and takes little extra time for the procedure. The cost of this pacemaker is almost the same as previous available devices

Honored with PADMA BHUSHAN by president of india , Dr Kler is pioneer in cardiac electrophysiology in India . He was the one to implant first ICD (device to prevent sudden cardiac death) in 1995 and then heart failure device (CRT-P) and COMBO device (CRT-D)In India.

Evolving Sources of News for Journalists - "#MediaInsights Report"

IN · · Leave a Comment
IIMC
New Delhi:Research jointly conducted by 20:20 MSL and IIMC to study the changing patterns of news sourcing by journalists, especially with the advent of social media

In an attempt to demystify the rapidly evolving media universe in India and shed light on the growing appeal of new-age alternatives to the press release, 20:20 MSL (part of MSLGROUP, Publicis Groupe's strategic communications and engagement consultancy and the largest PR network in India) in strong collaboration with the Advertising & Public Relations Department of the Indian Institute of Mass Communication (IIMC) today launched the "#MediaInsights report", the first-of-its kind research to arrive at insights that would benefit India's media and communications industry.

When a piping hot story is on the anvil, only one in eleven journalists would use more than half the press releases that flood their email inboxes. Internal brainstorming sessions remain the most preferred among many, when it comes to exercising their grey cells in the newsroom. An emphasis on the diversity of content from the media has driven a series of compelling changes in the conventions of news sourcing among journalists and other media professionals. The public sentiments that are considered in constructing a story, the advent of the social media space as a hub for story ideas and extensive use of official websites to cull out authentic data are some of the most striking industry-wide changes that the report has highlighted.

Commenting on the report, Mr. Chetan Mahajan, Managing Director, 20:20 MSL said, "It is imperative for a next generation insights-driven agency to understand the media environment in order to be a strategic advisor to the stakeholders. We jointly conducted this research with IIMC, in order to get insights to address the gap between news-makers, news-consumers and the media. The report has successfully mapped the changes and brought out some compelling facts. I believe that this report will help PR practitioners in best aligning their strategies and tactics."

Mr. Sunit Tandon, Director General, Indian Institute of Mass Communication welcomed the report as "the first in a series of such studies that would be useful for all who are interested in understanding the rapidly changing contours and dynamics of the Indian news media sector."

The research was conducted with a stratified sample of 309 journalists nationwide cutting across age, gender, specialization etc. The report also focuses on several statistical data sets including the following:

(i) The ratio of stories produced by senior correspondents to those filed by young reporters
(ii) The frequency of social media usage in newsrooms, and
(iii) The reliance that journalists place on the Internet for sourcing news
(iv) The male versus women reporters' behavior towards different news sources

India Must Lead Global Sustainable Development Agenda Post Rio Plus 20 Summit

IN · · Leave a Comment
New Delhi, Delhi, India, Monday, July 02, 2012 -- (Business Wire India)
The Energy and Resources Institute (TERI) held a special media interaction on the challenges and opportunities in the context of global sustainable development agenda post the recently concluded Rio Plus 20 Summit today. Director General of TERI, Dr. R K Pachauri, Executive Director Dr. Leena Srivastava and Distinguished Fellows Nitin Desai, Ambassador Chandrashekhar Dasgupta & Dr. Prodipto Ghosh, reflected on the various issues raised at the Summit and underscored the need to tackle unsustainable means of production and consumption.

The recent culmination of the Rio Plus 20 Summit has once again fuelled socio-political debate on the challenges and opportunities that lie ahead on the road to sustainable development. While a number of environmental activists and NGOs have expressed deep disappointment with the outcome of this major event, several governments view it as a victory and vindication of their own specific stands. TERI’s distinguished speakers said the need of the hour was to engage concerned stakeholders to arrive at a consensus on critical issues such as equity between developed and developing nations in achieving sustainable development.

Speakers felt there were two ways of following the outcome of Rio+20. One was to conclude that nothing of substance and certainly no agreements were produced by this conference. The second view would be to conclude that the ‘future we want’ as documented as the outcome of the conference clearly provided a wide canvas of concepts and directions that the world could possibly adopt towards the pursuit of sustainable development.

Speakers also pointed out that at the Summit, the developed countries had attempted to rollback principles of equity, which were an integral outcome of the Rio Conference held in 1992. However, lauding the strong and united stand taken by the developing nations at the Summit, Dr. R K Pachauri, Director General, TERI, said: “It is time India takes the lead and takes charge of the intellectual leadership of the sustainable development goals (SDGs) raised at the Summit.” Dr. Pachauri further added, “The final produced at the culmination of the Rio Summit had enough substance for each community and stakeholder to pick up elements to promote the discourse of global sustainable development.”

Speaking at the occasion, Executive Director, TERI, Dr. Leena Srivastava said: “The final document recognised the need of clean energy to achieve sustainable development. Emphasis of the Summit was also on boosting education and capacity building to meet sustainability targets.”

Ambassador C. Dasgupta, a Distinguished Fellow at TERI, said: “The road ahead would focus on the evolution and scope of sustainable development goals (SDGs) and their relation to the Millennium Development Goals.”

Nitin Desai, Distinguished Fellow, TERI, said: “The interest and participation of corporate entities and the scientific and academic communities at the Summit was far greater at the Rio Plus 20 Summit than ever before. This is so because the trend is no longer limited to green consumerism, but to green investors.”

Dr. Pradipto Ghosh, Distinguished Fellow, TERI, opined: “The focus on sustainable production and consumption is the main point that has emerged from the Summit. Strong unity among the G-77 group to protect their individual development space has also come out strongly through the conference.”

Speakers also highlighted relevant opportunities to enhance the scope of the sustainable development agenda:

i) Profiling the elements of a green economy in the context of sustainable development and poverty eradication. The concept of green economy would need to take into account links between financing, technology, capacity building and national needs for sustainable development.

ii) Enunciating sustainable development goals and motivating of stakeholders, including business and industry to set in place a reporting system involving sustainability.

iii) Establishing regional, national, sub-national and local initiatives to implement the provisions of the Rio+20 document.

iv) Dealing effectively with the challenge of climate change and problems such as deforestation and loss of biodiversity as well as the problems of desertification, land degradation and drought.

v) Evaluating pathways to sustainable consumption and production.

About The Energy and Resources Institute (TERI):
